Insurance
An important part of maintaining your oral health is visiting the dentist regularly. In efforts to make great dental care accessible to everyone, we accept and are Preferred Providers for several dental PPO plans and discount plans. We also welcome patients covered by the Arizona Health Care Cost Containment System (AHCCCS).
Our experienced front office team will submit all insurance claims for you, but be aware that your co-payment is due at the time of treatment. We will make every effort to help you receive your full insurance benefits, but the patient is still responsible for the account. If your policy has not paid within 30 days, give our office a call.
New Patient Forms
If you are new to Orangewood Family Dental, you can now download and preprint your new patient forms. The FORMS are available in PDF format For your convenience, we recommend printing and completing these forms at home. When it’s time for your appointment, please remember to bring your completed forms with you. If you have any questions or need to reschedule, please give us a call.
Payments and Financing
Before you receive treatment, we will give you a breakdown of all fees and estimated costs for the recommended treatment. If you do have insurance coverage, your co-payment will be due at the time of treatment. At Orangewood Family Dental, we accept Visa, MasterCard, American Express and Discover credit cards. We can also offer financing through CareCredit and Sunbit.

Sunbit offers flexible financing options so you can afford treatment on your terms. We can find a financing solution that works for you.
